How we read quality‑system evidence

Evidence comes before opinion

Each piece leads with what the data show and where they come from. Interpretation follows and is marked as such.

Control limits mark signals

Exhibits borrow the conventions of statistical process control. A center line (CL) and upper and lower control limits (UCL, LCL) are drawn from the series itself. Points outside the limits are marked as signals with a shape and a word as well as a color.

Scroll sideways to see the whole schematic.

  1. Number. Kicker "Exhibit 1", numbered in sequence through the piece.
  2. Finding title. A full sentence that states what the exhibit shows.
  3. Unit line. Measure, unit, population and period.
  4. Data tag. "Illustrative data" whenever values are placeholders.
  5. Center line and limits. CL solid; UCL and LCL dashed, drawn from the series itself.
  6. Signal. Only points outside the limits, marked with a filled shape and a word, never color alone.
  7. Note and source. Always present, below a hairline, with the data table and permalink.
Anatomy of an exhibit. Schematic; the live example is Exhibit 1 in the flagship report.
  • Every exhibit is numbered, carries a full-sentence title stating its finding, a unit line and a source line.
  • Exhibits that use placeholder values carry an “Illustrative data” tag, and their source line says so.
  • Charts include a data table alternative and a text description for screen readers.
